How are users added or removed from a MS Access .laccdb lock file?

I understand that the .laccdb file created by MS Access is used to prevent modifications to the database by other users. I don't understand how the list of users in the .laccdb is generated and managed. When I open the lock file using Notepad++ I usually see multiple users and computers names listed. Sometimes the same user is listed more than once or a user that has already closed the database is still listed.

How is a name added or removed from the .laccdb lock file? Is the order of users in the file important?
